So, what is the indicator that the post thumbnail is actually a header, and not a regular thumbnail image? An extra custom field, naming convention? Anyway to have both a custom header and a thumbnail?

In this theme they are one in the same — the header space is where we use the post thumbnail image. There may be a better way of doing so, but WordPress Trac would be the venue for that discussion.
